From complex wound care and disease management education to physical rehabilitation after surgery, Precision Health Care develops individualized care plans under the supervision of physicians and provided in the comfort and security of patients’ homes. We also provide supportive services to assist with Activities of Daily Living by our team of reliable and experienced Caregivers.

We are locally and privately owned and operated since 2011. Our clinicians and caregivers live within the communities we serve.
